Currently, the User Profile Plugin only supports textfields and textareas to synchronize user directory attributes into. This means Yes, it is not possible right now to synchronize any kind of image to Confluence.
But we are going to implement new field types for future releases. There is an issue regarding this question so you are able to vote on it to increase it's importance.pictures from a user directory to Confluence. You can find more information on how it works in the Administrator Guide.
